Patient Care Overview
We have collected resources to support care for members in our network. You will find behavioral health services, maternal child services, health education resources and Condition Care/Population Health Program support.
Behavioral health
Our behavioral health provider network includes psychiatrists, psychologists, social workers, psychiatric nurses, other mental health providers and employee assistance health care professionals. Our facility network includes hospital inpatient units, residential treatment centers, partial hospitalization programs, and intensive outpatient services and programs.
Maternal child services
When it comes to our pregnant members, we are committed to keeping both mom and baby healthy. That’s why we encourage all our moms-to-be to join Taking Care of Baby and Me®, a comprehensive case management and care coordination program for expectant mothers and their newborns.
Health education resources
We advocate a community approach to health education and outreach activities, and have a proven record of collaborating with community and faith-based organizations, advocacy groups and other community resources.
Condition Care/Population Health/Clinical Health Promotion
Our Condition Care/Population Health Program is based on a system of coordinated care management interventions and communications designed to support physicians and other providers in managing members with chronic conditions.
